Revisiting these cases raised a red flag about the overreliance on automated identification systems or manual methods using outdated databases, without critical analysis by the microbiologist.<\/p><p>This story is revisited in a <a href=\"https:\/\/www.youtube.com\/watch?v=CdxKmVoPjhU\" target=\"_blank\" rel=\"noopener\">documentary<\/a> that examines the Brazilian outbreak and the process of re-evaluating the isolates, featuring expert testimonies and behind-the-scenes insights into the investigation that led to the pathogen\u2019s reclassification.<\/p><p><strong>From the Environment to the Hospital<\/strong><\/p><p>Originally found in soil and plant microbiomes, <em>Phytobacter<\/em> fits within the One Health framework. This profile positions the genus as a potential reservoir of antimicrobial resistance, with direct implications for therapy and infection control.<\/p><p><strong>What the Quality Exercise Revealed in Practice<\/strong><\/p><p>In an External Quality Assessment (AEQ) conducted by <a href=\"https:\/\/controllab.com\/en\/\" target=\"_blank\" rel=\"noopener\">Controllab<\/a> in partnership with <a href=\"https:\/\/lacen.saude.pr.gov.br\/\" target=\"_blank\" rel=\"noopener\">Lacen\/PR<\/a>, an anonymized sample containing <em>Phytobacter diazotrophicus<\/em> was sent to hundreds of laboratories. Most participants misidentified the microorganism as <em>Pantoea<\/em> spp., particularly when using automated systems based on biochemical panels with outdated databases.<\/p><p>The error is not merely taxonomic. It can lead to underestimating clinical risk, hinder outbreak detection, and influence therapeutic decisions. MALDI-TOF platforms with updated databases performed better, underscoring the importance of keeping these tools current.<\/p><p>Detailed results of this exercise are presented in a new technical publication (<a href=\"https:\/\/www.flipsnack.com\/revistalaesehaes\/laes-haes-279-sysmex\" target=\"_blank\" rel=\"noopener\">Laes &amp; Haes, issue 279, p. 72\u201384<\/a>), including a comparative analysis of identification methods and implications for routine laboratory practice.<\/p><p><strong>Automation Requires a Critical Eye<\/strong><\/p><p>Automation has undeniably advanced microbiology, but the data show it cannot replace expert interpretation. Inconsistent profiles\u2014such as isolates identified as <em>Pantoea<\/em> with a positive indole test or an identification probability below 98%\u2014should be treated as warning signs.<\/p><p>For Dr. Marcelo Pillonetto, scientist at the Laborat\u00f3rio Central do Paran\u00e1 (Lacen\/PR) and professor at PUCPR, the case of <em>Phytobacter<\/em> illustrates a growing challenge. \u201cRelying solely on equipment results can mask clinically relevant pathogens. Integration of automation, updated MALDI-TOF, and critical microbiologist analysis is what ensures a safe and actionable diagnosis for the clinical team,\u201d he says.<\/p><p><strong>Pathways to Risk Reduction<\/strong><\/p><p>Experts highlight priority actions for laboratories:<\/p><p>\u2022 Periodic updates of automated system and MALDI-TOF databases; <br \/>\u2022 Critical review of isolates from sterile sites identified as <em>Pantoea<\/em> spp.; <br \/>\u2022 Use of molecular methods for confirmation in critical cases; <br \/>\u2022 Ongoing training in taxonomy and microbiological interpretation.<\/p><p><strong>Accurate Diagnosis Means Patient Safety<\/strong><\/p><p>The high rate of misidentification of <em>Phytobacter diazotrophicus<\/em> reveals a real risk for epidemiological surveillance and patient care.
